In this quick tutorial, we are going to look at the field types that you can have in your form template. For that we have prepared for you a form here that we want to use. The first question is actually this right here. How satisfied are you with your overall job? So of course I’ll click right here. I’ll go ahead and put the question over here. I can decide to add a description. It is your choice to add a description for the field or not For this purpose, I’ll go ahead and utilize this description. Now notice that the description is actually for the type of field. In this case, this is a rating field. So I’ll go right over here and I’ll select rating. Now the system does preload, the existing rating values, but as you can see, you can very much edit that. (00:56): So in my case here, I’m actually going to edit it by utilizing the suggested text. So of course, very dissatisfied. I guess I can just go with dissatisfied here and then neither agree or disagree. And then of course data on the other side on the positive side will be very satisfied and so on and so forth. So of course, as you can see here, I’ll change, agree to satisfied, and then I’ll change the last from strongly agree to very satisfied. So that’s how you do that. And then of course, as we saw in our previous video, you can select the metric that goes with this part of question. The other piece you can do is you can choose to allow explanation. And if you’re not sure what this is, just put your cursor over here and we’ll explain. You can also choose to make this mandatory or not. (01:50): And basically it means that the user cannot ignore this question, they have to respond to it. And then that basically creates my first field. And so we can go to the next field here, rate your performance over the last quarter, and this is going to be a gradient type question. So let’s go ahead and click like so, alright, and this is going to be a grading type question, so I’ll do that. And you can choose a star or dot rating grading style. And then of course you can choose the grade, scale five or whatever you want. And then I’m going to go ahead and make this mandatory. So as you can see, I am actually going down my list of questions here and I can continue to do this. The next one is called the ENPS. So again, you can just select the question type right over here. (02:46): The system basically preloads the responses for you. So no need to do much on that. And hit add. And then we’ll go to the next type. This is on a scale of zero to 100. So this is called a range. Alright, so I’ll click right over here. Scale of zero to 100, I’ll select range. And here’s a cool thing you see right here. We give you a default of 10, but guess what? For this pillar question, it needs to go up to 100. So I’ll go ahead and push it to 100, add in there. And let’s go down a list. This is a question that is a single choice question. So basically it’s a multiple choice, but you want to accept only one response. Okay? So when you go over here, you go single choice, and then of course you add your responses in there. (03:39): So right over here I’ll show you just how simple it is to add these values. So click on this and add it because I have word, it’s copying word code. So just be it. Pay attention to that stuff. Sometimes word does add some additional stuff that is not required. And anyways, but as you can see, I am adding these here. I’ll stop at three if you will, just so that we can move on with the rest of the field types. And again, decide if you want to make that mandatory, allow explanation what have you, and hit that. And go ahead and add it in there. The next question is a multiple choice, multiple selections. So basically we have several choices, but we do want them to respond and select multiple answers if that’s what they want to do. So unlike the single choice responses, whether there’s only one answer if you will, in this case, they can select multiple answers. (04:45): So this is what this type of field is. It’s a multiple choice, multiple selection type of a question. So I’ll go ahead and enter a few and then we can go to the next one just to give you all a chance to see just how comprehensive this system is when it comes to your survey or reviews and other types of engagement needs. Alright, and then I’m actually going to go ahead and since I’m here, I’m just going to go ahead and copy the next question and that will be a short text question. So I’ll go ahead and add my next question right here. So again, this is the question over here, short text and I can choose to make it mandatory. You don’t need to have anything else for that. You just add a question. A short text is basically limited in terms of the amount of words that can be typed in and a long text is longer. (05:42): So if you think that a question requires of them to provide more intensive response than select a long text and basically hit save, that is literally how simple it is. How about that? We’ll go ahead and preview this. I’ll click on the save so that I can save my template right quick. And then I’ll go right over here and I’ll preview so that you can see just how beautiful this is. Look at that. So there’s the question, there’s the description, there are the responses, and these are the different types of questions. So in a few minutes, we have created a form template and now we can deploy this into a survey. We can deploy this into a performance review or a pulse survey or some other need.
